Pool Application Submission requires a completed form along with Photos of every eligible member of your household. Passport Photos ARE an acceptable photo format.                                                                                            Short Term or Long Term Guests of your residence ARE NOT Eligible to receive a Pool Pass..

For Application Submission:

Standard Method: Return the completed application along with a 1-inch by 1-inch face photo of every listed household member (with the name on the back) to the Life Guard on duty at Pool Entrance

Electronic Method: Request an electronic copy of this form by emailing TotaBoard@TotaNews.net. Once completed, send it along with a 1-inch by 1-inch face photo of every listed household member (with the name indicate for each photo) to the Thousand Oaks Photo Editor at: DMRFR89@gmail.com

Scanned Electronic Method:A paper completed form may be scanned along with the associated photos and sent to the Thousand Oaks Pool Editor at: DMRFR89@gmail.com

Additional Information required for RENTER Applicants ONLY: 

Property Owner's Waiver of Pool Access Rights 

To be Completed by the Thousand Oaks Homeowner of the Rented Residence
